what is the chip?
The Community Health Improvement Plan is a collaboration of partners across the Tulsa area working to improve health outcomes in: Built Environment, Educational Attainment, Food Environment, or Health Care.


We invite you to nominate exceptional partners and projects as a CHIP Champion. Each month a CHIP Champion is selected and the good work they’re doing is highlighted across the community.

  • Quarterly CHIP Meetings
  • Alliance groups in each of the four focus areas
  • Micro-grant funding from Pathways to Health
  • Grants and partnership opportunities with other funders.
